Physical Exam Form | Please upload a copy of your child's most recent physical exam to your CampInTouch Account. You may use this form, or a copy from your school. This form is to be completed by a Physcian. |
Medication Authorization | For campers and U.18 staff one form for EACH medication (prescribed OR over the counter). Bring to check in with the medication in it's original container. |
Medical Information for International Campers | Read this form if you have an international camper. |
Waiver - Medical Exemption | Use this form if your child’s doctor recommends a medical exemption from required immunizations. |
Child Protection: CT Grooming Info Sheet | The Office of Early Childhood reccomends parents/guardians read this tip sheet prior to putting your child in another persons care. |
